- ਡਾਕਟਰ ਦੀ ਲਿਖੀ ਦਵਾਈ ਵਾਲੀ ਪਰਚੀ (Prescription slip) ਤੁਹਾਡੇ ਵਟ੍ਸ ਐੱਪ ਨੰਬਰ ਤੇ ਭੇਜ ਦਿੱਤੀ ਜਾਵੇਗੀ l
- ਤੁਹਾਡੀ ਪਰਚੀ (Prescription slip) ਉੱਪਰ ਅਗਰ ਕੋਈ ਡਿਸਪੈਂਸਰੀ ਦੀ ਦਵਾਈ ਦਸੀ ਗਈ ਹੋਵੇਗੀ ਤਾਂ ਡਿਸਪੈਂਸਰੀ ਤੋਂ ਫ੍ਰੀ ਲੈ ਸਕਦੇ ਹੋ l (ਭਾਈ ਲਾਲੋ ਜੀ ਚੈਰੀਟੇਬਲ ਹਸਪਤਾਲ)

- ਮਰੀਜ਼ ਦਾ ਵਜ਼ਨ ਕਿੰਨਾ ਹੈ ਅਤੇ ਲੰਬਾਈ ਕਿੰਨੀ ਹੈ l
- ਪੁਰਾਣੀਆਂ ਬਲੱਡ ਰਿਪੋਰਟ, ਐਕਸਰੇ, ਅਲਟਰਾਸਾਉਂਡ ਅਤੇ ਹੋਰ ਕੋਈ ਰਿਪੋਰਟ l
- ਕਿਸੇ ਦਵਾਈ ਤੋਂ ਅਲਰਜੀ ਹੈ l
- ਪੁਰਾਣੀ ਕੋਈ ਬਿਮਾਰੀ ਦੀ ਦਵਾਈ ਜੇ ਖਾ ਰਿਹਾ ਹੈ l
- ਹੋਰ ਜੇ ਕੋਈ ਗੱਲ ਦੱਸਣਾ ਚਾਹੁੰਦਾ ਹੈ l
